You will receive your online paperwork right after booking your appointment. Please fill this out at least 24hrs prior to your appointment.
Insurance verification will be done as a courtesy if requested but patients are responsible for knowing their coverage prior to the visit. Dr. Amy is in network with MODA, REGENCE BLUE CROSS BLUE SHIELD AND PACIFIC SOURCE (NOT OHP).If you are out of network you can request a super bill for visits to self submit to your insurance company. We accept HSA/HRA and cash. New patient visits (60 min) are between $150-$195 and follow up visits (15-30min) are between $65-$95 depending on how detailed the treatment is.
LOCATION:
We are located in the heights with on street parking. Please don't come in without texting (971 258 1802) upon your arrival. We have an open concept office and may be with another patient.
SCHEDULING:
For future appointments you can book online @ amylesliedc.com. If you know you may need follow-up appointments you can book those in advance as my schedule does fill up quickly. There is a $50 late cancel fee for appointments cancelled within 48 hrs of your scheduled appointment time. Exceptions include health or family emergencies. There is a $90 fee for no-shows.
Please bring your insurance card and wear comfortable clothing that allows for movement.

How much does one Appointment cost?
New patient visits are between $150-$195 and follow up visits are between $65-$95 depending on how detailed the treatment is. Cash or check is preferred as credit card fees are hard on small businesses.
Many other chiropractic offices don’t work with insurance companies to avoid the hassle of it all.
We believe that you already pay a lot of money for your healthcare so we want you to get as much out of it as possible. We also believe that it is important for us to work within healthcare to change their policies to recognize the value of our services.
